Application Checklist

 

STEPS TO COMPLETE THE EXAM APPLICATION

STEP 1   Download the Certification Guide to learn about eligibility requirements, instructions for completing the application, the exam structure, exam preparation, scoring, certificates, fees, and more!
STEP 2 After confirming that you meet the eligibility requirements, if you have an existing account with The Commission, log in HERE. If you need to create an account, do so HERE.
STEP 3 Once logged in, select "My Account" at the top of the page and then select "CDMS Exam Application".
STEP 4 Complete the application in its entirety and then choose "Checkout and Make Payment".

INFORMATION NEEDED TO COMPLETE THE APPLICATION
If you are applying with a current, active RN license, you will need to know:

  • Name of license
  • Who issued the license
  • License number
  • State that issued the license
  • Date you became licensed
  • Expiration date of the license 

OR

If you are applying with a baccalaureate or graduate degree, you will need to know:

  • Degree
  • Area of concentration
  • Name of institution
  • Year of graduation

All applicants will need to provide:

  • Current employment setting
  • Employer(s), start and end dates to fulfill eligibility requirements
  • Supervisor(s) name(s) (Spelled correctly and listed the same way as listed in The Commission’s database, and be mindful of maiden/married surnames).
  • Supervisor(s) current email address(es) (If audited, they will be contacted via email to verify employment).
