Community Health Workers (CHWs) are increasingly recognized as vital members of interprofessional teams. They serve as connectors with deep levels of trust with the communities they serve and many have shared lived experience and knowledge of community-based resources.  They work as important members of the team to coordinate care and meet the holistic needs of clients. 

The Commission today is expanding to meet the education and certification needs of a broader range of client advocacy professionals to ensure that Americans have access to a team-based, future-ready workforce. Today’s client advocacy professionals include case managers, disability management specialists, community health workers, social and behavioral health workers and others who advocate on behalf of the individuals they serve. While professional training, roles, titles and work settings may differ, client advocacy professionals help individuals navigate care, access resources and achieve better outcomes in complex health, social, employer and community settings. The Commission is committed to certification excellence and lifelong learning for these individuals. 

In this webinar you will learn how CHWs contribute to client-centered care, particularly in underserved populations, and how integrating CHWs into care teams supports better outcomes, stronger engagement and improved health equity. The session will also highlight opportunities for Board-Certified Case Managers to collaborate effectively with CHWs, laying the groundwork for future workforce partnerships.

Learner Outcomes:

  1. Explain how CHWs contribute to patient-centered and client-centered care models.
  2. Identify strategies for integrating CHWs into interprofessional teams to partner with and extend the reach of Board-Certified Case Managers.
  3. Discuss the unique value CHWs bring to underserved and vulnerable populations.
  4. Recognize opportunities for Board-Certified Case Managers to advocate for CHWs as part of broader workforce development and client engagement strategies.

This statistic has been repeated by news outlets, government talks, and even a Netflix series. But where does this comparison originate? It's rooted in a noteworthy study led by Learning Network™ guest speaker, Dr. Julianne Holt-Lunstad, and her dedicated research team. 

Did you know social connection is directly tied to one's health? 
Our lack of social connection is creating an “epidemic of loneliness” that threatens our individual and collective health. That’s not just a general observation: It comes directly from the US Surgeon General. The 82-page report from his office - Our Epidemic of Loneliness and Isolation - articulates the problem and identifies solutions. 

The healing effects of social connection and community in the context of well-being are the centerpiece of this Learning Network webinar featuring  Julianne Holt-Lunstad, Ph.D., professor of psychology and neuroscience and director of the Social Connection & Health Lab at Brigham Young University.
